In BDSM, the intent in sexual humiliation isn't abuse in the absolute sense of the word. One isn't trying to cause anyone permanent injury or lasting harm… that isn't the point (again, I'm talking about humiliation the fetish here - while it doesn't have to be overtly sexual in form, the motivation is sexual stimulation). The intent is to provoke intense feelings of shame and embarrassment in the given person, through actions and situations that are intentionally humiliating. Not so one person can just hurt another, but so both people can take pleasure in what is being done. While some experience pleasure through physical pain, for others the pain of embarrassment and shame alone gives them feelings of excitement and euphoria (though infliction of physical pain in a femdom session, for example, can easily cause these same feelings of shame due to the dynamic of the domination / submission scene). Usually though, there will be a blending of many different triggers that will lead to sexual fulfillment of this kind.
 
For example, I mentioned the use of a whip earlier. It's application in BDSM not only causes physical pain for the one on the receiving end, it also invokes degrees of fear, vulnerability, submission, etc., along with the knowledge that what is being enjoyed is not approved of by society - it's something others would be shocked by and think is wrong. The sadist needs someone to abuse, while the masochist needs someone to do the abusing… as things proceed, the increasing suffering of the masochist stimulates the sadist more, while making the masochist feel more vulnerable and excited in turn.
 
For some people, sexual humiliation and that which is otherwise related to the fetish, can actually be somewhat cathartic, purging them of deeply held emotions in need of an outlet that is safe and controlled. That's not to say that in all cases this is completely healthy, for there are people who take these things to levels beyond what one should, for reasons that are not at all good. But when within responsible bounds, involving sensible people with their eyes open, the participants are normally much happier for it.
